Do you accept my insurance?

Cox Physical Therapy is contracted with most major insurance companies including Blue Cross, Blue Shield, Aetna, MPHW, SAG and Medicare. If we are not contracted with your insurance company, we will bill through your PPO benefits. It is a good idea to be aware of your insurance company's coverage regarding physical therapy benefits. If you are unsure, we will gladly find out for you and let you know prior to your visit.

What should I bring to my first appointment?

Bring your doctor's prescription, your insurance card and driver's license. We will email you the necessary forms prior to your first visit. If you are being treated for your lower extremity, shorts would be appropriate. If you are being treated for your upper extremity, a tank top or camisole is appropriate. Spinal patients would be most comfortable wearing loose fitting clothing.

How long is each appointment?

Please allow at least 60 minutes for the first session. Following visits may range from 45 to 75 minutes.

Is there parking available?

There is plenty of two-hour street parking near the building. There is a parking lot attached to the building. We do not validate.
